Youth Information Sheet

The Youth Information Sheet is a 2-page form that must be completed and signed by a parent once a year. It provides important contact and medical information to St Martin's youth leaders.

Youth Event Permission Slip

The Youth Event Permission Slip must be completed and turned in to the youth leader prior to each youth event.

Youth Event Guest Permission Slip

The Youth Event Guest Permission Slip is a 2-page form used by guests that wish to participate in St Martin's youth events. It must be completed, signed by both the parent and participant, and given to the youth leader.

Medical and Photo Release

The Medical and Photo Release is a 1-page form used for VBS and other events to allow St. Martins to seek medical attention for and take photos and videos of participants at St. Martin events.
