Important forms The Youth Information Sheet is a 2-page form that must be completed and signed by a parent once a year. It provides important contact and medical information to St Martin's youth leaders.
The Youth Event Permission Slip must be completed and turned in to the youth leader prior to each youth event.
The Youth Event Guest Permission Slip is a 2-page form used by guests that wish to participate in St Martin's youth events. It must be completed, signed by both the parent and participant, and given to the youth leader. Youth Event Guest Premission Slip
